&lt;p&gt;After a little more looking, if you are creating a template off of the “Blank Site” site definition you need to also add another FeatureSiteTemplateAssociation for the TemplateName of STS#1. &amp;nbsp;The Blank site is a little special in that regards.&lt;/p&gt;

&lt;p&gt;I understand why you may want to have a choice field instead of the single line of text for the Task column, however it looks like they just re-named the &amp;quot;special&amp;quot; Title field to Task, so there is no good way to remove it from the list, nor can you change the type of field that it’s in.&lt;/p&gt;

&lt;p&gt;Since SharePoint can’t find out someone’s manager automatically, you would need to create a custom list with an employee column and a manager column, both people columns that your workflow would look into. &amp;nbsp;If you had all those entries setup, in a step you could do a lookup into your managers custom list where employee = created by in your current item, then set that manager column in a workflow variable. &amp;nbsp;Once you have that variable you should be able to put that in your TO field for the email (it should return DOMAIN\username which will get translated to the email address once it hits your email server).&lt;/p&gt;
&lt;p&gt;With that same logic you should be able to do the same type of lookup for when the manager approves the item to email the employee back. &amp;nbsp;The biggest issue is getting a list to track that information since none of that is accessible (WSS or MOSS) through SPD&lt;/p&gt;

&lt;p&gt;If you are trying to hide the &amp;quot;Service Request&amp;quot; name field, that is the out of the box title field, just renamed... and because everything with that item is derrived from that Title, you really shouldn&amp;#39;t hide that field because it should get filled out.&lt;/p&gt;
&lt;p&gt;That doesn&amp;#39;t mean you couldn&amp;#39;t auto-populate that field with their name and date of the request (or any other information you want to standardize on) by using the calculated value of the &amp;quot;default value&amp;quot; option.&lt;/p&gt;
&lt;p&gt;As for the ability to limit what people see, take a look at my response to &amp;quot;I Love SharePoint&amp;quot; where I specify how to utilize Item-Level permissions on any list.&lt;/p&gt;
